This page outlines the new benefit, being offered by the office, available to anyone who needs to wear prescription safety glasses at work.
To qualify you have to be employed for more then 12 months OR it has to be a minimum of 2 years since your last claim.
You are eligible to receive 50% of the cost of your glasses back from from the office up to a total reimbursement of $300 (if your glasses are $600 or less you will receive a reimbursement of 50%… if your glasses are over $600 you will receive a maximum of $300).
Remember through our benefits provider, OpenCircle there is an additional benefit for prescription glasses. An addition $150 available for your prescription safety glasses.
You can also use your training fund allowance to go towards this expense so that gives you potentially up to $850 to use towards your prescription safety glasses.
You scan your receipt and send it to Jim Baskerville and he will fill out your expense claim form and send it and the receipt to Chris Ell. Chris will, once approved, forward this on to payroll, after which it should appear on your next paycheck. (This process can sometimes take a little while, so please have some patience)
Most eye glass stores should be able to direct bill to CanadaLife. If not you will have to submit the receipt for reimbursement as you would for anything else. (ie: Dentist, Chiropractor…)
Simply follow the same procedure as normal, only use the receipt from your eyeglasses purchase. For those not familiar with the process, simply download the Training Fund Claim Form by clicking HERE follow the procedures as laid out on the form, attach your receipt and submit the form.
